Mexican Caldo de Papa recipe?

I'm looking for a Caldo de Papa (mexican potato soup) recipe. How I remember it is it has chunks of cheese, potato and "rajas" (jalape�o slices). It has milk, or cream, in it, but it's not necessarily a creamy soup, it's more of a broth. Anyone know how to make this? Thanks.
